Federal Bureau of Prisons Form BP-A0629 Visitor Information Form submitted by Petitioner Adrienne
Rockenhaus to Warden Eric Rardin and Counselor Grimm at FCI Milan on or about November 5, 2025,
together with covering email from Petitioner's rockenhaus.com address stating this was her “third
attempt” to submit the application. Form requests visitation privileges with Respondent Conrad
Rockenhaus (Reg. No. 39400-480), identified on the form as her husband.
Relevance:
This sworn federal form is contemporaneous documentary evidence that Petitioner, three months
before filing the ex parte PPO, was actively and persistently seeking to visit Respondent in federal
custody. The BP-A0629 is a sworn document; Petitioner's repeated submissions (the cover email
states this was her “3rd attempt”) are affirmative, volitional acts seeking increased contact with
Respondent, not protection from him. The continued characterization of Respondent as her “husband”
in a sworn federal submission, coupled with the persistent pursuit of visitation, is inconsistent with the
fear-based allegations in the ex parte PPO petition filed only 107 days later.
